How is Lifeway Plus different?

Relationship.
Our long-term relationship with you is built on mutual trust and respect. I want to know details of your life that affect your motivation to be healthy.

Service.
The medical industry is not known for service or convenience. We deliver service that will knock your socks off. Office appointments will be available on the same or the next business day with me. Gina will answer your phone calls promptly. I will see you on time.

Non-office based care is readily available.
Many office visits do not actually need to occur. Wouldn't it be great to come to the office for a thorough exam once a year, and then have follow-up via phone, or Internet video meetings? Wouldn't it be great to have secure e-mail communication with each other? Wouldn't it be great to receive a text message from my nurse that your new prescription has been sent to the pharmacist? Technology has made many things possible for us in our lives, and we will utilize them to make medical care more accessible.

Facilitator.
Coordination of your care in the complex healthcare system is very difficult, but we will talk with you, family members, and other providers between visits as the need arises to ensure that small details do not fall through the cracks.

Does my insurance cover the annual fee?

No. The annual fee pays for non-covered services such as Internet video visits, and cutting-edge cardiovascular screening tests.

How does my insurance work with Lifeway Plus?

We will continue to bill your health insurance plan for office visits, and routine testing. Most health-insurance plans are compatible with your membership in Lifeway Plus. We will be happy to discuss your specific health plan with you. You will still need health insurance to cover all other healthcare visits with other providers, and hospitals.

What is the annual membership fee?

$360 per year for ages 27 - 49, or about $1 per day.
$720 per year for ages 50 - 69, or about $2 per day.
$1080 per year for patients 70 and over, or about $3 per day.

Children of members will not have to pay a fee.

Do my company benefits cover the annual fee?

Maybe. The annual fee may be paid through most employer section 125 plans and is compatible with most Flexible Spending Accounts (FSA), Medical Savings Accounts (MSA), and Health Savings Accounts (HSA). We are happy to send you a receipt that you can submit to your plan administrator to see if it will be approved for reimbursement. We do know that it would need to be submitted at the end of the year, as the plans will not reimburse prospective expenses. You cannot pay the fee with your FSA debit or credit card. Reimbursement for your annual fee will have to be obtained through your FSA claim submission process.

Do I have to pay the annual fee all at once?

No. The annual fee may be paid monthly through an automatic draft with your checking account. It can also be paid semiannually, or annually by credit card.

What happens when Dr. Thomas is away on vacation, or otherwise unavailable?

I will always arrange for appropriate coverage with the providers at Lifeway Family Physicians. I may also be available via phone to handle your problem.

Will Dr. Thomas see family members or friends who visit me?

Yes. Family members under 27 years of age will be seen without the annual fee. Family or friends who are visiting you will be seen with standard office visit fees.

How does Lifeway Plus work with specialists in the hospital?

We offer enhanced coordination of specialty care. With this program, we will have the time to make sure that all appropriate information is given to your specialist. I will communicate with the hospital doctors before, during, and after hospitalization when it occurs.

Is this a concierge practice?

No. A concierge practice typically is limited to 200 patients, typically charges $1500 - $3000, and office visits last for 30 minutes to two hours.

What happens if I am not satisfied?

We will work to correct the problem, but if you remain unsatisfied, you may opt out of Lifeway Plus at any time since the membership fee is paid monthly.

How do I join?

You can become a member of Lifeway Plus by filling out a Membership Agreement and Payment Authorization form, and mailing them to our office. You may also become a member by meeting with Deirdre. She will personally take you through the process of joining Lifeway Plus, and answer any questions you may have.
